Stubborn Fat

Do you eat well and work out, but still struggle to shift or target the lower belly, hips, inner thighs, or chin? If yes, you're not alone. When most of your body responds and these areas don't, it can feel like they're working against you.

Not all fat is the same. Some are a pinchable layer under the skin and tend to hold on the hardest. Other fats sit deeper, around the organs, and respond well to diet and exercise. And some of what looks like fat is bloating or loose skin. Many people have a combination; the first step is working out what you're dealing with.

What’s actually happening?

Fat isn't one uniform thing, and the human body mainly contains three types of fat cells:

  • White fat: stored beneath the skin and around the organs, including the belly, thighs, arms, and buttocks. It serves as an energy reserve, but too much is linked to obesity.
  • Brown fat: present in smaller amounts in adults, commonly around the neck and shoulders. It burns stored energy and helps regulate body temperature, activating in cold temperatures to warm you up.
  • Beige fat: a combination of white and brown fat cells. Like brown fat, it stores energy but can burn it when exposed to cold, and it also helps regulate body temperature.

Fat can also be described by its location and what it does. This is where stubborn fat comes in, since stubborn fat is primarily white fat:

  • Essential fat: found in the brain, bone marrow, nerve tissues, and cell membranes. It protects your organs and plays a role in hormone regulation, vitamin absorption, and temperature regulation. It is essential for a healthy body, and it isn't the fat people are trying to get rid of.
  • Subcutaneous fat: the soft, pinchable layer under the skin, commonly found on the belly, hips, thighs, arms, and chin. It is the type most likely to hold on despite diet and exercise.
  • Visceral fat: sits deep in the abdomen around the organs and tends to respond more readily to diet and exercise.

Fat cells carry receptors that tell them either to store energy or to release it. In common trouble areas, such as the lower belly, hips, inner thighs, and chin, the fat tends to carry more of the "store" signal. These areas also tend to receive less blood flow, so less of the stored fat is carried away and used.

What contributes to stubborn fat?

  • Aging: fat tends to increase while muscle mass and function drop, even in people who stay active.
  • Fat cell biology: the balance of "store" and "release" receptors varies by area and from person to person.
  • Genetics: inherited traits can largely affect how and where your body stores or distributes fat.
  • Hormonal changes: declining estrogen around perimenopause and menopause can shift where the body stores fat. This is one reason many women notice more belly fat during this time even without gaining weight overall.
  • Lifestyle habits: eating more calories than you burn, a diet high in processed foods and sugar, alcohol, stress, and a mostly sedentary routine all contribute.
  • Pregnancy: can lead to fat accumulation around the belly and inner thighs that may persist afterward.

What Can Help

Doctor and client in a body assessment consultation at a clinic

At Cutis Medical Laser Clinics, we start with a body assessment by our doctors. They look at the area, your skin, your weight history, current medications, and goals. The choice of treatment is a clinical decision, not just an aesthetic one.

Aesthetic

  • CoolSculpting Elite: a US FDA-cleared fat freezing system that uses controlled cooling to crystallize fat cells. The body then clears the treated cells gradually through the lymphatic system.

Its C-shaped applicators complement the body's curves, and dual applicators allow two areas to be treated at once. It can be used on the abdomen, flanks, thighs, bra and back fat, upper arms, buttocks, under the chin, and jawline.

  • ZLipo: a CE-certified fat freezing system with 360-degree cooling handpieces in a range of sizes. It triggers apoptosis, or natural cell death, without damaging the surrounding skin and tissues, gradually reducing fat layer thickness.

Lifestyle

  • Eat a balanced diet with fewer processed foods and added sugars.
  • Combine cardio and strength training, and, if it suits you, interval training.
  • Choose a routine you enjoy, since consistency matters more than intensity.
  • Limit alcohol consumption.
  • Manage stress and get enough sleep.
  • Consider a consultation with a doctor or dietitian for a plan suited to your goals.

It can treat both broader areas and smaller or harder-to-reach ones, including the belly, chin, arms, inner thighs, and knees. Two handpieces can be used together to treat more at once.

  • Acoustic Shockwave with Cryolipolysis: combines fat freezing with acoustic shockwave therapy. The latter works on the connective tissue rather than on fat cells. Fat freezing reduces fat thickness in the treated area, while shockwave supports lymphatic drainage, collagen production, and skin texture.
  • Cutis Medical Gym: a subscription-based approach that gives you access to a range of US FDA-cleared and CE-marked non-invasive body shaping technologies. Treatments/devices are chosen around your goals, whether that's fat reduction, muscle toning, or skin tightening. It's designed to complement a healthy lifestyle.

What to Expect

The procedure: Sessions typically take 30 to 45 minutes per area, and dual applicators allow two areas to be treated at once. You may feel a cold, pulling sensation at first that settles quickly.

Side effects: Redness, swelling, and temporary numbness or tingling in the treated area are common. They usually resolve within a few hours to a few days, and there's generally no downtime.

Results: Change is gradual because the body takes weeks to clear the treated fat cells. Early improvements may show from around 3 to 4 weeks, and most of the visible result from a cycle is usually apparent by about 8 to 12 weeks.

Treatments needed: Some areas need only one or two sessions, while others need more, spaced about a month or more apart.

Acoustic shockwave: Typically done as a course of sessions, often once or twice a week. Whether shockwave is done on the same day as fat freezing or as a separate session is a clinical decision, and your doctor will outline the plan for you.

Note: Treated fat cells are cleared by the body, but neighboring fat cells can still expand if you gain weight. This is why these treatments suit people whose weight is stable.

When Treatment Isn't the Answer

Patient and doctor in a clinic consultation.

Fat freezing is a body contouring treatment, not a weight-loss solution. It reduces fat thickness in the treated area, but it doesn't change your overall body weight, visceral fat, or metabolic health.

It's generally ideal for people at or near a stable weight, in good general health, with localized, pinchable fat that hasn't responded to diet and exercise. It isn't suitable for fat deep in the abdomen, and it doesn't tighten loose skin or improve cellulite on its own.

If your weight is still changing (like being on weight management medication), it is best to wait until your weight settles. Results are less predictable while treatment areas are changing. If your main concern is overall weight (rather than a specific area), a weight management consultation may be the better first step.

FAQs

Is fat freezing a weight-loss treatment?

No. It reduces fat in certain areas and is meant to complement a healthy lifestyle. It cannot replace regular diet and exercise.

How many sessions will I need?

It depends on the area, the thickness of the fat, and your goals. Some areas need only one or two sessions, while others need more. Your doctor will outline a plan after an assessment.

Is there downtime?

There's generally none. Temporary redness, swelling, and numbness or tingling in the treated area are common and usually settle within hours to days.

Can the fat come back?

Treated fat cells are gone and cleared by the body. But nearby and remaining fat cells can still expand if you gain weight. Keeping your weight stable helps maintain your results.

What's the difference between CoolSculpting Elite and ZLipo?

Both use fat freezing, with different applicators and handpieces. Which one suits you depends on the area being treated and how it fits the device, and your doctor will recommend one after a consultation.
