THE SCIENCE

Two components, two biological roles, one formulation.

The clinical rationale for a hybrid filler is grounded in peer-reviewed dermatological and plastic surgery literature. Each component plays a distinct and complementary role within the dermal and subcutaneous tissue.

Composition

The injectable comprises a cross-linked hyaluronic acid gel matrix at a concentration of 20 mg/mL, combined with calcium hydroxyapatite microspheres sized between 25 and 45 micrometres at approximately 55 percent by weight. A small concentration of lidocaine is included to support patient comfort during injection.

The two-component mechanism

The hyaluronic acid component delivers immediate volume and provides the matrix that carries the calcium hydroxyapatite microspheres into the tissue at the planned depth. 

As the hyaluronic acid is gradually metabolised by the body over several months, the calcium hydroxyapatite microspheres remain in place, where they have been shown to act as a scaffold around which the patient's own fibroblasts produce new collagen. This biological process is referred to in the literature as neocollagenesis.

Calcium hydroxyapatite is a well-characterised biomaterial that has been used in aesthetic medicine for over two decades, originally as a stand-alone biostimulator. Its mechanism of activating dermal fibroblasts to produce new collagen type I has been documented in published preclinical and clinical studies. 

The hybrid formulation pairs that biostimulatory action with the immediate soft-tissue support of cross-linked hyaluronic acid, which clinical observation suggests also aids tissue integration of the calcium hydroxyapatite microspheres.

WHO MAY BE A SUITABLE CANDIDATE

A hybrid filler is most often considered for patients whose facial concerns extend beyond a single category. Common indications include:

Step 01 · Consultation — Assessment and planning

You will sit with one of our doctors for a thorough consultation. This includes a review of your medical history, an assessment of your skin and facial structure using AURA skin analysis, and a discussion of your concerns and aspirations. 

You will then receive a structured treatment plan covering short, medium and longer-term considerations.

Step 02 · Treatment — The procedure itself

The injection itself takes approximately thirty to forty-five minutes. Topical anaesthetic is used to support patient comfort. Your doctor will use either a needle or a cannula, depending on the area and your individual anatomy. 

Some swelling, mild bruising or tenderness in the injection sites is common in the days that follow.

Step 03 · Aftercare — Continuity of care

You will receive specific aftercare guidance. The treating doctor manages all post-treatment concerns personally, with WhatsApp contact available for any questions during clinic hours. 

We do not consider the treatment complete until you are satisfied that any concerns have been properly addressed.

Multiple peer-reviewed studies have evaluated hybrid hyaluronic acid and calcium hydroxyapatite formulations using objective imaging methods. Key published findings include:

  • Increased dermal thickness. A prospective single-centre study published in Plastic and Reconstructive Surgery: Global Open in 2023 reported a statistically significant increase in dermal thickness on ultrasound at 90 and 120 days following a single treatment (Bravo et al., 2023).
  • Increased tissue viscoelasticity. A study published in the Journal of Cosmetic Dermatology in 2023 documented increased density, viscoelasticity and firmness of the treated tissue on elastography at 60, 90 and 180 days, suggestive of new collagen formation (Urdiales-Gálvez et al., 2023).
  • Real-world safety and outcomes. An Italian retrospective study following 129 patients over a nine-month period reported favorable patient-rated and physician-rated outcomes with only minor adverse events resolving spontaneously.

What is a hybrid filler?

How does a hybrid filler compare to a thread lift or to surgery?

A hybrid filler is not a substitute for a surgical procedure such as a facelift. Surgery addresses skin redundancy and tissue position in a way that injectables cannot.

Compared with a thread lift, a hybrid filler does not place a physical lifting device in the tissue; instead, it provides volume and is intended to stimulate collagen renewal over time.

How quickly will I see results?

The hyaluronic acid component provides an immediate effect that is visible from Day 1, although swelling in the first days can mask the final result. The collagen-related changes from the calcium hydroxyapatite component build progressively over several months. 

Published imaging studies have shown measurable changes in dermal thickness and tissue density at 90 to 180 days following treatment.

Will the result look natural?

A natural-looking result depends on careful patient selection, treatment planning and injection technique. At Cutis, your doctor assesses your facial anatomy, your overall plan and your aesthetic preferences before any treatment is administered. 

The intent of a hybrid filler is to support your existing facial structure, not to alter it. Where a natural result is at risk because of patient factors or anatomy, your doctor will say so directly during consultation.

How long do the effects of a hybrid filler last?

The visible effects of a hybrid filler treatment may last between 12 and 18 months, depending on the individual, the area treated, the volume injected, and lifestyle factors. 

Some patients experience effects beyond that period, particularly from the collagen-related component. Your doctor will discuss expected longevity with you during consultation.

Is the treatment painful?

Topical anaesthetic is used to support patient comfort during the procedure. The formulation itself contains a small concentration of lidocaine. Most patients describe the sensation as manageable. Your doctor will use either a needle or a cannula, depending on the area and your individual anatomy.

What is the recovery period?

Some redness, mild swelling, bruising or tenderness in the injection sites is common in the first 24 to 48 hours, with swelling sometimes taking up to a week to fully resolve. 

Most patients return to their usual activities soon after treatment, although your doctor will give specific guidance based on the area treated.

How do I prepare for the treatment?

For one to two weeks before your treatment, your doctor may ask you to avoid medications and supplements that can increase bruising risk. These include aspirin, ibuprofen, fish oil, and high-dose vitamin E, where it is medically safe to do so. 

Alcohol should be avoided in the 24 to 48 hours before treatment. If you have a history of cold sores, please mention this during consultation, as antiviral prophylaxis may be advised. Specific preparation instructions will be given to you during your consultation.

What should I avoid after the treatment?

For the first 24 to 48 hours, your doctor will typically advise avoiding strenuous exercise, prolonged heat exposure such as sauna, hot yoga or very hot showers, and aggressive facial massage or pressure on the treated area. 

You will also be advised to avoid other facial procedures such as lasers, peels and microneedling for a defined period, and to follow a gentle skincare routine. Specific aftercare guidance is provided in writing.

Safety and considerations

What are the potential side effects?

The most common side effects are mild and temporary: redness, swelling, tenderness, bruising and itching at injection sites, typically resolving within 24 to 48 hours, with swelling occasionally taking up to a week. 

Less common effects can include lumps or nodules under the skin. Rare but important risks of any injectable filler include vascular events (where filler enters a blood vessel), infection, hypersensitivity reactions and material migration. 

Your doctor will discuss these risks in detail during consultation.

Who is not suitable for a hybrid filler?

According to the manufacturer's instructions and good clinical practice, hybrid filler is generally not suitable for:

  • Patients with a known hypersensitivity or allergy to any of the filler components, including, hyaluronic acid, calcium hydroxyapatite, or lidocaine.
  • Patients with an active skin condition, infection, or inflammation in or near the treatment area.
  • Patients who have a tendency to develop keloid scars or hypertrophic scars
  • Patients with impaired wound healing.
  • Patients with bleeding disorders, or those taking anticoagulant (blood-thinning) medications without appropriate medical clearance.
  • Patients with a history of severe allergic reactions or anaphylaxis.
  • Patients who are pregnant or breastfeeding.
  • Patients under 18 years of age.

Areas not treated with hybrid filler

Hybrid filler is also not intended for use in the following areas:

  • Lips
  • Perioral area (around the mouth)
  • Glabella (the area between the eyebrows)
  • Periorbital area (around the eyes)

Are the effects reversible?

This is an important question, and the honest answer is partial. The HA component of a hybrid filler can be partially addressed by injecting hyaluronidase, the same enzyme used to dissolve traditional hyaluronic acid fillers. 

The CaHA component, however, cannot be enzymatically dissolved; it is gradually metabolised by the body over time as new collagen forms around it. 

For this reason, careful patient selection, conservative dosing and meticulous injection technique are particularly important with a hybrid filler.

Will my skin sag or look worse when the effects wear off?

A common concern with any volume-restoring treatment is whether the skin will appear worse once the effects fade. With a hybrid filler, the calcium hydroxyapatite component is intended to support new collagen formation in the months following treatment. 

Published imaging studies have shown sustained increases in dermal thickness during the follow-up periods studied. Long-term data on what happens to the skin once all effects have fully faded remains incomplete; the available evidence does not point to deterioration below baseline, but individual responses do vary.
